What Is the SMAS Layer?
The SMAS is a very thin fibromuscular layer located at approximately 4.5mm depth between the subcutaneous fat and muscle layers, which is difficult to reach with lasers. It is a fascial layer beneath the subcutaneous fat where facial expression muscles, connective tissue, fascia, and blood vessels are located. As the skin sags with aging, the SMAS layer sags along with it. If only the skin is pulled without addressing the SMAS layer, the lifting effect is diminished and can result in severe scarring or pixie ear deformity.

Retaining ligaments are soft tissue structures that connect from the bone through muscle, fascia, subcutaneous fat, and all the way to the dermal layer, playing a crucial role in preventing the skin from sagging.
